Members wanted for transitions and integration focus groups

The Community Services, Transitions and Integration (CSTI) team is seeking physicians to participate in one of two focus groups in December to help inform partnership development at the CWC PCN.

The sessions will be held the evenings of Thursday, December 5, and Thursday, December 12 (December 12 session is now full), at the network’s Main Office with a limited number of spots. Participants will be remunerated for their time.

As a physician in Calgary, your experiences and insights are invaluable in helping us identify priorities and opportunities to improve the health services that affect you and your patients. These focus groups will explore the challenges you regularly face when working with healthcare partners across the wider system.

Over the past two years, the CSTI team conducted one-on-one interviews with over 60 members to gain a deeper understanding of the challenges physicians and patients face when navigating the health system.

The information was analyzed and reported on in May 2019 and June 2018 in support of the provincial objective of Strong Partnerships and Transitions of Care.

These new focus groups are intended to further investigate existing and emerging trends. Particularly:

  • What patient populations or diagnoses need stronger community supports
  • Who you identify as key partners in providing patient care outside your own clinic and PCN
  • What opportunities for improvement do you see in the flow of information between yourself, your clinic and community partners/specialty programs
