UPDATE! Membership model agreements

New membership agreements will be distributed the week of Monday, January 14, to coincide with the completion of an updated FAQ about the membership model.

The agreements will be emailed to you using DocuSign — a secure, cloud-based tool – to provide access on your smartphone or any online device.

You do not need a DocuSign account to review and sign the documents. When you receive the email titled "Please DocuSign: CWC PCN Required Membership Agreements," simply click the “Review Documents” link and follow the prompts.

The comprehensive FAQ is based on questions submitted by members during orientation sessions in December and covers the membership model, our team members, operationalizing PMH, the transition period, data sharing, digital health and governance. Once finalized, it will replace the FAQ currently housed on the New Membership Model page of the Examiner.
