Excel Health is seeking full- or part-time physicians and specialists.

Outreach Emergency Shelter Physician  December 3, 2020

Outreach Emergency Shelter Physician – Ref. No. 20076
Calgary Drop-In & Rehab Centre
Term | 0.2 - 0.4 FTE

The Alex: Changing Health. Changing Lives. The Alex Community Health Centre is a not-for-profit organization that is changing how we look at health care in Calgary. Our health, housing and food programs support our most vulnerable neighbours, tackling tough health and social issues head-on. Using a multi-disciplinary team approach, we focus care on assessment, intervention and promotion of all aspects of health. We embrace a model of health care that is accessible, responsive and participatory. In addition to the Community Health Centre, we run a Seniors Health Centre, a Youth Centre, two Mobile Community Health Buses, one Dental Health Bus, four Housing First Programs, and Community Food Centre.

What we do and why we do it:

Family Physicians at the Shelter Medical Clinic provide walk-in and appointment-based family practice at the Calgary Drop-In Centre in a non-judgmental, harm-reduction focused, holistic, and opportunistic manner. As part of the DI health services team, physicians are required to understand the social determinants of health as they pertain to this vulnerable and underserved population, and to provide full-scope medical care with a trauma-informed and harm reduction perspective. Physicians are required to have strong skills in addiction and mental health, as well as full scope medical care in a complex shelter population. Methadone prescribing is required.

Reimbursement is via alternative-relationship plan (ARP) with booked appointments typically 30-60 minutes long; physicians are expected to participate in team-based, multidisciplinary care.

The Calgary Drop-In Centre (the DI) is more than emergency shelter. They provide essential care as well as health services, employment training, and housing supports to people who need help. The goal of their programs and services is to connect people to permanent housing that meets their individual needs. Rooted in community and fueled by kindness, the DI proudly serves as part of the Homeless-Serving System of Care.

COVID-19 Response in Shelter
In addition to primary care at the DI, Family Physicians provide on-site care and on-call support during COVID outbreaks at the shelter. In the event of a COVID outbreak at the DI, shelter residents are occasionally required to isolate due to case or close contact status. Physicians are required to support referrals and continuity of care from the shelter environment to ASIS. In addition, residents of the shelter with ILI are required to self-isolate while living at the DI. Family physicians are required to work closely with Public Health and DI staff in contact tracing, locating, isolating, and managing COVID cases in the shelter population. These duties will be shared amongst the physicians at the DI and The Alex’s ASIS team. This is a term position until March 2021 offering a flexible FTE of 0.2 – 0.4.

What you will do:

  • Full scope family practice; most participants are between the ages 24 - 65
  • COVID-19 shelter outbreak response
  • Special skill sets are encouraged and preferred (i.e. opiate agonist therapy, managed alcohol, transgender medicine, procedures, IUD insertions, emergency/urgent care, mental health, sports medicine, etc.)
  • Work effectively and collaboratively with psychiatrists, nurses, mental health providers, social workers, client resource specialists, pharmacy, etc.
  • Facilitate and support allied health team members to work to full-scope of practice
  • Familiarity with Community Treatment Orders
  • Flexibility in regards to patient scheduling for same-day, walk-in appointments
  • Openness to outreach work in patient homes and permanent supportive housing buildings with support from the clinical teams
  • Respond to acute concerns as appropriate for a family physician in an outpatient setting (i.e. irregular behaviour, drug overdoses, active suicidality)
  • As appropriate, support Alex-based research and evaluations initiatives to support organizational theory of change and clinical practice
  • Feel comfortable in completing a Form 1, as required
  • Be accountable to ARP billing processes, including complete shadow billing for direct and indirect client care via EMR processes

Physicians at the Alex may expect:

  • The ability to practice comprehensive medicine in an ARP-billing environment; appointments are shadow-billed to Alberta Health via a combination of SOMB and ARP-specific codes, however, are generally more in-depth than a comparative fee-for-service medical practice
  • Administrative support – patient panel management, management of daily appointment flow through clinic (including bookings, confirmations, rooming, appropriate diagnostics and investigations), facilitation of referrals, and other services
  • Integrated medical practice with nursing and other onsite allied health, including psychiatry, pharmacy, optometry, chiropractor, acupuncture, mental health therapy, diabetes educators, dietitian, and others
  • Onsite laboratory (phlebotomy)
  • Collaborative approach with social resource team to work with clients facing socioeconomic barriers

What you bring:

  • Good standing with the College of Physicians and Surgeons of Alberta
  • Prior experience working with vulnerable populations (e.g. homeless serving sector, refugee health, remote outreach medicine, or similar) an asset
  • Compassion for clients with challenging socioeconomic and life situations
  • Ability to multi-task in a fast-paced environment
  • Ability to manage medical IT systems – EMR, Netcare, medical references • A satisfactory criminal record clearance is required
  • Standard First Aid – Level C CPR & AED are mandatory
  • Knowledge of the United Nations Declaration on the Rights of Indigenous People (UNDRIP) and the Truth and Reconciliation Commissions (TRC) 94 Calls to Action an asset

If you are as passionate as we are about making a difference in people’s lives, please submit your resume to jobs@thealex.ca and be sure to include the job title and reference number in the subject line.
