Patient Care Teams

Patient Care Teams are professionals who are located in member practices. These team members include nurses, behavioural health consultants, centralized social workers and patient care coordinators.

Patient care coordination is a new service that will provide member physicians with assistance in gaining a better understanding of patient needs by arranging well-organized visits when patients need to be seen and helping to track the practice’s achievements in improved patient care. This service will also help provide support for disease prevention and screening as well as focusing on mental health and chronic disease management.
